How do GLP-1 programs set their prices?

Why is the advertised GLP-1 price often not what you pay?
Short answer: As of October 2026, four things push the real cost above the advertised price: first-month prices that rise later, billing every 28 days instead of monthly (13 charges a year), required memberships of $74 to $149 a month, and prices that rise with your dose. Belle charges one flat monthly price at every dose.
Introductory prices
Some programs advertise a first-month or starter-bundle price. Remedy Meds charges $179 for the first semaglutide supply, then $299. TrimRx charges $199 for the first month, then $299/mo. Orderly Meds offered new customers a 2-month starter bundle at about half its standard rate.
Billing that is not monthly
A price charged every 4 weeks (a 28-day supply) means 13 charges a year, not 12. At $299 per fill that is $3,887 a year instead of $3,588. Remedy Meds and Fridays charge every 28 days, and Brello Health billed $499 every 11 weeks with a 3-month minimum. WeightWatchers Med+ lists 28-day supplies for brand-name medication.
Memberships and dose increases
A required membership can add $74 to $149 a month. Dose-based pricing means your cost can climb as you move toward a maintenance dose, which is where most people spend the longest.
