Belle Rewards Terms

Belle Rewards gives Belle patients account credit ("Belle Credit") for purchases, milestones and a few simple steps in their care. These terms explain how it works. They're part of Belle's Terms of Service, and if the two ever conflict on Belle Rewards, these terms control.

1. Who's included

Every patient with a Belle account is enrolled automatically. There's nothing to sign up for and no fee. You can use your credit while you have an active or paused Belle plan.

2. What you receive

Cash back on eligible purchases: 5% of the amount you pay, on every eligible order. Expires 365 days after it's issued.
Three months with Belle: $25, once. Expires 365 days after it's issued.
Six months with Belle: $50, once. Never expires.
One year with Belle: $100, once. Never expires.
Birthday reward: $20, once a year. Expires 365 days after it's issued.
Log your initial weight: $5, once. Expires 365 days after it's issued.
Log your first shot with Belle: $5, once. Expires 365 days after it's issued.
Join the Belle Community: $15, once. Expires 365 days after it's issued.

"Once" means once per patient, for the life of your account. A reward is issued to one Belle account only, even if you have more than one. Expiration dates are subject to the 90-day rule in section 5.

3. How each reward works

Cash back. You receive 5% of the amount you actually pay for an eligible order, after discounts and promo codes. The part of an order you pay with Belle Credit doesn't receive cash back. Credit is issued once your payment goes through.

Eligible purchases are paid orders for Belle treatments and products. Shipping, taxes and gift cards aren't eligible.

Milestones (three months, six months, one year). Milestones count the time you've been a Belle patient, starting from your first paid Belle order, including orders placed before Belle moved to its current patient portal. Pausing your plan doesn't reset the count. If you cancel and later come back, the count starts again from your return. If you'd already reached one or more milestones when Belle Rewards launched, you receive each of them within 60 days of launch.

Birthday reward. You receive $20 in Belle Credit around your birthday each year, based on the date of birth on your Belle account. Changing your date of birth doesn't create an extra birthday reward.

Initial weight and first shot. You receive $5 the first time you log your weight with Belle, and $5 the first time you log a shot with Belle, in your Belle patient portal. If you logged either one before Belle Rewards launched, your next log after launch qualifies.

Join the Belle Community. You receive $15 when you join the Belle Community using the same email address as your Belle account. The community is free.

4. Using your credit

Belle Credit is applied automatically to your next Belle order or plan renewal, and it can cover the full amount.
Your balance and your next milestone are shown in your patient portal.
Credit is used oldest first, so credit that expires soonest gets used first.

5. When credit expires

Credit expires on the date shown for each reward in section 2. Separately, if you have no active or paused Belle plan for 90 consecutive days, any unused credit expires, including credit that otherwise wouldn't expire.

6. Refunds

If an order is refunded, in whole or in part, the cash back from that order is taken back in proportion to the refund. Milestone, birthday, logging and community rewards aren't affected by refunds. If you paid for a refunded order partly with Belle Credit, that portion goes back to your account as Belle Credit, not cash.

7. What Belle Credit is not

Belle Credit has no cash value. It can't be exchanged for cash, refunded as cash, or paid out when you close your account.
It can only be used on Belle orders.
It can't be sold, transferred or combined between accounts.
It's a promotional credit, not a gift card, stored-value card or payment instrument.

8. Your care comes first

Belle Rewards never affects medical decisions. Whether a treatment is prescribed, and at what dose, is decided only by a licensed provider based on your health. Receiving or holding credit doesn't guarantee a prescription, a refill or any particular treatment. If a provider doesn't approve an order, no cash back is issued for it.

Belle doesn't give rewards for reviews, testimonials or social media posts.

9. Government healthcare programs

Belle Rewards isn't available for any purchase paid for, in whole or in part, by Medicare, Medicaid, TRICARE or any other federal or state healthcare program. Where a law prohibits or limits the program, it doesn't apply to the extent of that law.

10. Fairness

Belle may withhold or remove credit, or close a Belle Rewards balance, if it was received through fraud, abuse, error, duplicate accounts, or anything that breaks these terms or Belle's Terms of Service. If credit was issued by mistake, Belle may correct your balance.

11. Changes to the program

Belle may change reward amounts, add or remove rewards, or end Belle Rewards. We'll post updated terms on this page and change the "Last updated" date. If Belle Rewards ends, we'll give at least 30 days' notice, and credit already in your account stays usable until its expiration date.
