Tesamorelin side effects and what's normal

Updated Oct 8, 2026From the Belle care team

Tesamorelin has the most side effects of Belle's peptides. Most of them are uncomfortable rather than dangerous, and there is one important exception at the end of this article.

Common and usually temporary

  • Water retention and puffiness. Common with growth hormone peptides, and not dangerous on its own.
  • Joint aches. Common, not dangerous, and usually easier after the first few weeks.
  • Tingling or numbness in your hands, a carpal-tunnel type feeling. It comes from the same fluid shift. Common, not dangerous.
  • Vivid dreams. Frequently reported, and it follows from dosing at night and sleeping more deeply.
  • Morning grogginess, particularly early in a cycle.
  • Anxiety and a faster heartbeat. Stronger and more common with tesamorelin than with sermorelin — that is the trade-off for it being stronger.
  • Lightheadedness or dizziness when standing up, especially if you are also taking a GLP1. Hydrating well, and electrolytes, such ash Hydrobelle, can be helpful to combat this.
  • Blood sugar changes, Because of increases in IGF1 and insulin, you may experience temporary increases and/or decreases in blood sugar. This is often transient, but if you are a diabetic, monitoring this closely to avoid too high or too low levels is important. Long term effect on insulin is neutral here, but the effect on visceral fat is positive for insulin resistance treatment.
  • Redness or irritation where you inject. Rotating sites is the fix.

The one important exception

If you have heart failure, tesamorelin is not a good fit for you, because of the fluid it makes your body hold on to.

New swelling in your legs together with shortness of breath is a same-day call. Do not wait for your next check-in — message us right away and we will get you in front of a provider that day.

Contact us if

  • You have new leg swelling, especially with shortness of breath — same day
  • A side effect is severe, persistent, or getting worse instead of better
  • The side effects are bad enough that you are thinking about stopping — there may be a dose adjustment that helps, and that is worth a conversation
  • Anything feels wrong enough that you are unsure about your next dose
  • You notice signs of an allergic reaction such as a rash, hives, or swelling. For trouble breathing or swelling of the face, lips or tongue, seek emergency care first, then let us know
